Questions About Your Georgia Medical Card?
Here’s what to have ready for your appointment and what to expect after approval.
What happens after my doctor approves me?
Your doctor submits your certification and information to Georgia’s patient registry. DPH checks your documents, then emails you instructions to pay the state card fee. Watch your inbox for payment instructions or requests for anything missing.
How much will I pay for my first Georgia card?
Your Elevate appointment is $110. Georgia DPH charges $30 for the state card, with an additional $3.75 service fee if you pay online. That brings the total to $143.75 when using online state payment. The state fees are separate from your Elevate appointment.
What ID and proof of residency should I bring?
Bring a current Georgia driver’s license or Georgia state-issued ID showing your current address. A passport, military ID, or out-of-state ID requires additional proof of Georgia residency.
A utility bill, phone bill, medical bill, or pay stub can be used if it shows your full name and current street address.
How long do I have to pay the state fee?
Submit your state payment within 60 days. If you miss that window, your card may be cancelled and you will need to contact your physician for reinstatement.
How and when will my Georgia card arrive?
Allow up to 10 business days after your payment is processed. DPH generally sends the card to your mailing address by UPS. A signature is required, so arrange for someone to receive it.
What if I use a P.O. Box or don’t have a secure mailing address?
Your card may be sent to the health department location nearest your residence for pickup instead. DPH will contact you when it is ready. Bring valid identification when you collect it.
Does a five-year card mean I only see the doctor every five years?
No. Georgia’s state card is valid for five years, but physician certification is required annually. Plan for a doctor’s visit each year. Renewal appointments with Elevate cost $90.
Who can help with state payment, delivery, or card status?
Once your provider has submitted your certification, contact the Georgia Medical Cannabis Patient Registry for state processing, payment, delivery, or card-status questions. Call (770) 909-2765, Monday–Friday, 8:00 AM–4:30 PM. You can also use the state’s card-status checker.
