What to Have Ready for Your Renewal
Check the date of your last physician certification and the expiration date printed on your card. Plan your certification visit each year, even while your five-year card is still valid.
If the physical card is due for renewal, allow time for the doctor’s visit, DPH verification, state payment, and delivery. DPH asks you to allow up to 10 business days after payment is processed to receive the card; that does not include the earlier steps.

How Much Does a Georgia Renewal Cost?
Your online renewal appointment with Elevate costs $90. State card fees are separate and apply when DPH issues a card. Follow the state’s payment instructions when a card is due. Elevate’s Full-Service option is not available in Georgia.

Missed Your Renewal Date?
If your annual certification is overdue: book a physician visit and bring your current card and ID. Tell the doctor when you were last certified and what has changed in your health or medications.
If your physical card has expired: tell the physician you also need to renew the card. After certification, follow DPH’s instructions for state payment and delivery. Booking an appointment does not extend the expiration date on your card.

Still Eligible for a Georgia Renewal?
Your renewal appointment focuses on your current health. You must remain a Georgia resident and have a qualifying condition. Tell your doctor what has changed since your last visit so they can decide whether to certify you again.
Georgia’s list includes conditions such as epilepsy-related seizure disorders, multiple sclerosis, Crohn’s disease, and intractable pain. Some conditions have age or severity requirements.

Georgia requires physician certification annually, even though the state-issued medical cannabis card is valid for five years. Keep track of both dates and plan your certification appointment each year.

If your ID does not show your current Georgia address, bring proof of residency with your full name and street address, such as a utility bill, phone bill, medical bill, or pay stub.
Do I pay the state card fee at every annual appointment?
The $90 Elevate appointment and the state card payment are separate. Georgia’s physical card is valid for five years, while physician certification is annual.
Pay the state card fee when DPH directs you to do so for card issuance. Do not assume that each annual certification visit means a new physical card.
How will I receive a new state card when one is issued?
After DPH verifies your information, it emails payment instructions. Submit payment within 60 days to avoid cancellation and the need to contact your physician for reinstatement.
Allow up to 10 business days after payment is processed. DPH generally sends the card by UPS with a signature required. If you use a P.O. Box or lack a secure mailing address, DPH may arrange health department pickup.
