Is the entire process online?
Yes, from start to finish, Elevate Holistics’ entire medical cannabis recommendation process is online. You will book the appointment from the website, as well as meet with your physician over the phone or computer. There’s no in-person involvement at all!
How much does an appointment cost?
Elevate Holistics, offers affordable pricing for our medical cannabis certification services. Depending on your state, our prices start at just $80. Plus, we offer significant discounts for veterans and Medicare recipients

How do I qualify for a medical marijuana card recommendation?
The qualification for medical cannabis varies depending on your location and the specific laws in place. Generally, patients must have a qualifying medical condition and provide medical documentation before being approved by a licensed medical professional.
You have to be at least 18 years old to qualify on your own; if you’re a minor, your parent or guardian must apply on your behalf. Please note that Elevate Holistics only serves minors in Missouri.
How long does the average appointment take?
How long your appointment takes will vary depending on the patient. However, most people will finish their consultations within about 10 minutes.
How do I renew my MMJ card?
Renewing your MMJ card with Elevate Holistics is easy. All you have to do is select the “RENEWAL Medical Cannabis Certification” option upon booking. Then, you will meet with the doctor for a lower price than first-time applicants.
Where do I download my recommendation/ condition form?
To download your recommendation or condition form after being approved, you will need to start by logging into your EH patient account at https://elevate.logointellect.com.
Once logged in, you need to select “APPOINTMENTS” from the options at the top of the page. From the list of appointments, you’ll want to select the most recent approved appointment.
Next, you’ll see the appointment information page. In the left-hand box, you will see a link in blue text that says “Recommendation/Condition Form.”
Select the link. Your device will prompt the download, and the file will save as a PDF into the specified download folder on your device where you can now access it when necessary.
Is my personal information secure?
Absolutely. Elevate Holistics is 100% HIPAA-compliant and never uses your personal information. We prioritize patient privacy and utilize secure, encrypted communication platforms to protect your personal information before, during, and after consultations.
Our secure site allows you to safely file your personal information with 256-bit encryption, versioning, and audit tracking.
Do I get my physical medical card through Elevate Holistics?
You can! However, it depends on the state. Let’s explain.
Elevate Holistics is a medical card recommendation platform: this means that you will get the medical recommendation needed for your MMJ card, not the physical card itself.
However, in states like Missouri, the medical marijuana program doesn’t provide patients with physical medical marijuana cards at all. Instead, you only receive a digital, downloadable version.
If you’d prefer a physical version of your MMJ card, Elevate Holistics can provide one for you! For a small fee, you can select our “physical card” add-on at checkout, and we will mail you a tangible version of your MMJ card for you to use.
What is an “add-on” service?
When booking your MMJ card appointment, you’ll have the option of also purchasing some of Elevate Holistics’ add-on services. These are services that go beyond just getting your medical recommendation: they’re aimed at making your experience more effortless than ever.
Here are some of our add-ons and what they entail:
Full-Service: We register your account with the state and complete your application for you — but we don’t submit it or pay the state fee. We’ll complete this within 48-72 hours of your appointment. Not applicable to all states.
Priority Full-Service: We register your account with the state and complete your application for you within just 24 hours — you just have to pay the state fee. You’ll receive one-on-one support from an agent to help you with any questions, concerns, or curiosities you may have. Not applicable to all states.
DHSS Physical Card: In MO, the state doesn’t send a physical card in the mail. Instead, DHSS approves you and uploads a PDF to your state profile. If you want a physical card, simply send us that PDF and your mailing address; then, we’ll print a card for you and mail it right to your door. This service is for Missouri only.
What does full-service include?
Full service is where we’ll create your account, ensure all information is correct, and fill out the information in the corresponding places.
We’ll get you registered with the state and completed up to the final payment portion! The only thing you have to do is pay the final fee and submit your application. We’ve got the rest handled.
Does a patient get a physical card or an electronic approval?
If you’re in Arkansas, Maryland, Oklahoma, Pennsylvania, or Virginia, you’ll receive a physical MMJ approval. The rest of the states provide e-cards.
What is the standard allotment and how can I get more?
Your standard allotment is going to vary depending on the state you live in. If you believe you require more MMJ than the allotment allows, you can talk to your physician about increasing it; however, there’s no guarantee that you can.
Where can I find my provider recommendation?
Please visit elevate-holistics.com within an hour of your appointment with the provider and select “view recommendation” to look at your recommendation form. This form includes the condition, approval date, and provider information.
What is Elevate's refund policy?
If you would like a refund, please submit a request through [email protected] for a cancellation or denial.
Please note that it may take 5-7 business days to process depending on your individual bank.
Can my provider sign additional documentation?
No. Whether for an employer, court, or other entity, we are unable to sign additional forms outside of the MMJ recommendation.
Where is your office located?
Elevate Holistics does not have a physical office! We are 100% virtual and excited to see you for your online appointment all across the country.
Can Elevate Holistics help me with my forms?
Yes! Feel free to give us a call at 816-400-4203 to request help. And, if you live in a state where we offer our full-service support, you can add this for a small fee and we’ll handle the entire process for you.
Can I get a replacement card from Elevate?
If you lose your MMJ card, how you replace it depends on where you live.
If you’re in Oklahoma, you will go to the website and fill out a simple form for replacement. In other states, the patient will contact the state itself.
If your physical card was purchased through Elevate Holistics as an add-on and is damaged within one year, we will replace it for free. If you lost it, we can replace it for a small fee of $20.
Can I use my coupon after I've booked?
If you’re using a Groupon, yes, we will refund you as long as we have a screenshot of your Groupon coupon emailed to [email protected].
Otherwise, coupons MUST be entered at checkout before clicking “purchase,” or you cannot use them.
