ToggleQuick answer, Yes! If you purchased a physical MMJ card with Elevate Holistics as an add-on and is damaged within one year, we will replace it for free. If you’ve lost your Elevate MMJ card, we can replace it, but we will require a small $20 processing fee.
Email us at support@elevate-holistics.com or call us at (816) 400-4203 and tell us what happened. If you’ve had your card for less than a year and it’s now damaged, we will replace it for free, no questions asked.
If you live in one of the other states where Elevate currently offers services, the first step you will need to take is to contact your state’s card authorization agency.
The process varies from state to state, but you will generally be asked to provide some type of identification documents, like your driver’s license or birth certificate, and pay a replacement fee. The agent will also likely ask you to submit an active MMJ recommendation (make sure to have it available beforehand).
For example, Oklahoma residents can call themselves lucky. All you have to do if you live in this state is go to the Oklahoma Medical Marijuana Authority (OMMA) website, log on to your account, fill out a replacement card online, and then just wait for it to arrive in the mail.

Here’s a short overview of the steps you need to take in different states to replace your medical marijuana card:
If you’ve lost your medical marijuana card in Arizona, you must file a request for replacement with the Department of Health Services within 10 working days of losing the MMJ card.
Then, go to the ADHS Individual Licensing page, log into your account, select Replacement Application, and follow the steps.
You must complete this form if you’ve lost your MMJ card in Connecticut.
Illinois no longer issues plastic cards, but you must report your lost or stolen MMJ card by contacting the Illinois Department of Public Health Division of Medical Cannabis within 10 days. You can then request a replacement from the Illinois Cannabis Tracking System.
Contact the Iowa Department of Health and Human Services by emailing them at medical.cannabidiol@idph.iowa.govreport or at 1-877-214-9313 to report your lost or stolen card.
If you lose your MMJ card in Maryland or you think it’s been stolen, you must report it to the Maryland Cannabis Administration (MCA) within 72 hours. You will also need an active MMJ recommendation to request a replacement. Elevate Holistics can help with that.
First, you must report the loss or theft to the Massachusetts Cannabis Control Commission within five business days. Then, you can submit a replacement request on the Cannabis Control Commission platform.
If you live in Michigan, you can request a replacement MMJ card by mail or online.
Medical marijuana cards are available online on the MMCP Licensing Portal.
The state of Missouri only issues digital cannabis cards. You can request a new one by accessing your Medical Cannabis Registry account.
You must report your lost or stolen MMJ card in New Jersey by reaching out to the state’s Medicinal Cannabis Program. You can then request a new card here.
Log in to https://my.ny.gov to request a new card and explain why you need it. The Department will analyze your request and issue a new MMJ card.
If you live in Oklahoma, you can easily request your replacement card on the OMMA website.
You can request a new MMJ card from the Profile Settings tab of your patient account. Remember that you must pay a replacement fee of $25 for the first replacement and $50 for each one after that.
You need to report a stolen or lost MMJ card to the OMC. You can request a replacement card by email at medcanwv@wv.gov.
