- If the minor doesn’t have a government ID, the parent/guardian’s ID must be used, instead. (This includes ID type, ID number, and a photo of the ID.)

- The parent/guardian must sign an additional consent form.

- You must provide a document showing your relationship with the patient. This could be a birth certificate, adoption records, legal guardianship paperwork, etc.

- The state application will include additional documents that your typical MMJ appointment wouldn’t:

- The parent/guardian must pay an additional $25 fee to be the patient’s caregiver, on top of Missouri’s $25 state fee.
The Steps
