If your medical condition is a qualifying condition the doctor will electronically submit the Physician Form to the state. You’ll have 30 days from this day to complete the application.  

Minors and patients that need someone to help with purchasing or using medical marijuana can have two caregiver cards. The requirements for this are they must be over 21 years old and a resident of Oklahoma. 

Proof of Oklahoma residency can be an Oklahoma state driver’s license or Oklahoma identification card. If you do not have either you can use a current utility bill, Oklahoma voter ID card, residential property deed, or a current rental agreement. 

Be sure to take your time filling out the application. If any information or documents are missing the application process will be delayed.  You’ll need to submit payment to complete the application. The standard cost is $100. If you’re not approved you will not get this money back.

If you are a 100% disabled veteran, qualify for Medicaid (SoonerCare), or Medicare enrollee you can get the application fee reduced to $20. You will need proof.

Within 14 business days from the submission date, you’ll receive an approval or denial letter. Your medical marijuana card will be included with your approval letter, and valid for two years.
