Step 3: How to Complete Your DHSS Medical Marijuana Forms Online
The DHSS offers the following forms to qualify for a medical card in Missouri. You will need one of these forms to be signed by the physician so you can submit to DHSS.
Your medical marijuana doctor can assist:
- Physician Certification Form (standard recommended dose of four (4) ounces per month)
- Physician Certification Form (requiring more than the standard four (4) ounces)
- Parent/Legal Guardian Form (patients under 18 years of age)

The DHSS website portal allows patients to register first and then submit their medical marijuana patient forms online.
Step 5: How to Submit Your Medical Card Application Online
Patients must apply within 30 days of their doctor’s recommendation.

- Click ‘Create Application’
- Click ‘New Patient Registration’
- Submit patient documents.
If errors are detected by the DHSS, the patient will have 10-days to correct the errors.
